Registration Procedures
1. Fill-in Family Last Name, Address, City, Zip and both the Home and Emergency telephone numbers. Please include the Parent’s Full Name when registering a minor. Additional registration forms are available at the Main Street Recreation Center and the Spring Avenue Recreation Center.

2. Payment must accompany your registration form. Payment can be made by personal check, payable to the Glen Ellyn Park District; or by cash when registering in-person; or by using Visa, MasterCard or Discover, providing all credit card information and signature. Your canceled check or credit card statement is your receipt. There is a $10.00 charge for returned checks and declined credit cards.

3. List your First and Second (optional) choice program codes. You will automatically be placed in the second choice if the first choice is already filled. If both choices are filled, you will be placed on a waiting list for your first choice. Please pay only for your first choice(s).

Any difference in cost will be handled either by refund or underpayment notification.

4. List the name of the program and matching fees.

5. List participant’s First Name in the proper space, corresponding to the desired class. This is the individual who will be attending the class.

6. Enter Social Security (*) for each participant by name. If you have a Universal Identification Card, please use the number on the card. If you do not have a Social Security Number, or prefer not to use it as your Universal ID number, please write the word “Assign” in the space provided. Our computer will assign a random number for you to use in future registrations.

7. List Male/Female and the Birthdate of the enrolling participant if you do not have a Universal Identification Number.

8. Registration postcards will be mailed indicating (a) you have been enrolled in the class or (b) that you have been placed on a waiting list. If you are moved off the waiting list into a class, you will be notified by post card or phone call.

9. The waiver form must be signed and dated. In the case of a minor, the waiver must be signed and dated by a parent or legal guardian.

10. Residents who submit their registration forms by the resident lottery registration deadline are entered into the computer and become part of the computerized lottery procedure. This procedure insures that: 1) fairness exists among residents and 2) residents do not compete with non-residents for class openings. Our registration lottery, done by computer, randomly selects registrants to be placed in programs. If the class is filled, participants are placed on a paid waiting list.

Park District Policy & Membership

Glen Ellyn Park District residents, through property taxes, make a financial contribution to the Park District operations. Nonresidents are charged higher fees for programs than residents to compensate for property tax disparity. Residents of Glen Ellyn who are not residents of the Glen Ellyn Park District or any other park district may receive all the privileges of belonging to the Glen Ellyn Park District by paying the equivalent of Park District taxes annually. This memberships allows for priority registration, pool passes, and any other services provided to our residents at the resident rate. For further information, please contact Jeanine Sesto at 630-858-2462, ext. 16.

Notice of Possible Changes In Brochure Listings
The policies, procedures, requirements, programs, fees and facility availability contained in this brochure represent Park District Program and Policies at the time of publication. Sometimes changes in the Programs and Policies are necessary or desirable after the brochure publication but prior to program commencement or facility operation. Brochure Delivery
The Glen Ellyn Park District brochures are delivered mid-July, mid-November, and mid-March. The Glen Ellyn Park District makes every effort to deliver to each resident household and assumes no responsibility for delivery.

Refund Policy
Please Read Carefully! Service Charge On Refund Requests
Due to the large number of refund requests received seasonally, the Park District assesses a service charge to ALL REFUNDS issued which are not a result of a Park District cancellation or waiting list placement. The service charge is $5.00 per program. Refunds for moving, injury/illness, or schedule changes will be prorated according to the date the request for a refund is made.

Satisfaction Guarantee Refund Policy

The Glen Ellyn Park District strives to provide participants with high quality recreation programs, activities and events. To encourage first time participation as well as repeat participation, the Park District has a refund policy highlighted by a satisfaction guarantee. This special refund policy provides us with timely feedback about dissatisfaction regarding our services. We can then take prompt action to improve services.

If you are not completely satisfied, tell us and we will either:
1. Invite you to repeat the class at no charge;
2. Allow you a credit that can be applied to another program;
3. Issue you a full refund.

Our guarantee is based on fulfilling our promise to deliver the published expectations describing the particular service you have used. A refund will be granted if you were dissatisfied with the experience due to circumstances under the Park District’s control. If you are not satisfied, we kindly ask you to give us your input or suggestions for improvement. Requests for the above mentioned options must be made before the midpoint of the program or within two weeks of a single meeting activity. All refund requests must be submitted on our form and returned to the Spring Avenue Recreation Center or the Main Street Recreation Center.

Satisfaction Guarantee does not apply to trips and special events with prepaid admissions and tickets.

Program Financial Assistance
The Park District recognizes that there are people in the community who are often unable to participate in recreation programs due to economic hardships. The District has established a recreation program financial assistance fund to assist families in the community. Financial assistance applications are available at the Main Street Recreation Center and the Spring Avenue Recreation Center. For further information, contact Kathleen Esposito, Superintendent of Recreation at 630-858-2462, ext. 21. All requests will remain confidential.
