You may fax your registration form to 630-942-8759. Payment
for program registration must be made with Visa, MasterCard or Discover.
Print clearly using black ink to ensure a clear transmitted
copy. Fax enrollment form only, including all credit card
information and signature.
Mailing-in saves time. No fuss and one of the easier ways
to register. Complete the enrollment form, credit card information
or a check for payment, list choices and all required information
and mail to:

The registration window is located on the second floor of the
Main Street Recreation Center, 501 Hill Ave. or the Spring
Avenue Recreation Center located at 185 Spring Avenue. Office
Hours: 8:30 a.m. - 8 p.m. Monday through Thursday, 8:30 a.m.
to 4:30 p.m. Fridays, and 9 a.m. to 12 Noon. on Saturdays.
Phone in registration begins three weeks after the resident
registration lottery deadline. Please have your Visa or MasterCard
number ready when you call.
